体征和症状

诊断
根据患者病史和体格检查,经常怀疑肩关节脱位的诊断。制作射线照片以确认诊断。大多数位错在X线片上显而易见,显示盂肱关节不一致。在标准AP射线照片上可能难以检测到后位错,但在其他视图上更容易检测到。减少后,通常重复X光片以确认成功减少并检测骨损伤。在反复肩关节脱位后,可以使用MRI扫描来评估软组织损伤。对于复发性脱位,逮捕测试(前部不稳定性)和窦沟征(下部不稳定性)是确定未来脱位倾向的有用方法。

脱位有三种主要类型:前,后和下。


在95%以上的肩关节脱位中,肱骨向前移位。[6]在大多数情况下,肱骨头在喙突过程中休息,称为亚喙突脱位。也可能发生下关节盂,锁骨下和非常罕见的胸内或腹膜后脱位[7]。

前脱位通常是由直接吹到或落在伸展的手臂上引起的。患者通常将他/她的手臂向外旋转并略微外展。

Hill-Sachs病变是关节脱位时关节盂边缘留下的肱骨头的影响。[5] Hill-Sachs畸形发生在35-40%的前脱位。当手臂处于内部旋转状态时,可以在正面X射线上看到它们。[8] Bankart病变是关节盂唇的破坏,伴有或不伴有骨碎片的撕脱。
