标题: 印戒细胞癌
印戒细胞癌（SRCC）是一种罕见的高度恶性腺癌，会产生粘蛋白。它是一种上皮恶性肿瘤，其特征在于印戒细胞的组织学外观。

原发性SRCC肿瘤最常见于胃的腺细胞（SRCC在90％的患者中起源于胃），而在乳腺，胆囊，膀胱和胰腺中较少见。 SRCC通常不会在肺中形成，尽管有一些报道。

原因
有些病例是遗传的，这些病例通常是由CDH1基因突变引起的，该基因编码重要的细胞间粘附糖蛋白E-钙粘着蛋白。 APC基因的体细胞突变也与胃SRCC的发展有关。

SRCC中尚未很好地研究其他风险因素在胃癌中的作用，例如盐腌食物，吸烟，自身免疫性胃炎。

形成机理
SRCC是去分化的腺癌，失去了细胞间相互作用的能力。

高分化腺癌通过黏附和紧密连接的缺失形成SRCC，这些黏附和紧密连接通常将粘蛋白MUC4和致癌受体ErbB2分开。当MUC4和ErbB2能够交互时，它们会触发激活循环。结果，ErbB2 / ErbB3信号传导途径被组成性激活，细胞间相互作用消失，形成了印戒癌。 ErbB2 / ErbB3复合物的本构作用也可促进细胞生长。

转移
胃印记细胞癌的转移方式与肠型胃癌的转移方式不同。 SRCC肿瘤通常见于腹膜，并且还已知扩散至肺部和卵巢的淋巴渗透，从而形成克鲁肯伯格肿瘤。也有胃癌转移到乳房并形成印戒细胞的病例。一项研究表明，当在乳腺肿瘤中发现印戒细胞时，也应考虑存在胃癌。

SRCC大肠癌的研究比较了富含粘蛋白的SRCC肿瘤和贫粘蛋白的SRCC肿瘤。他们得出的结论是，后者更经常表现出不利的组织学特征，例如淋巴管浸润，静脉浸润和神经周浸润。

治疗
化学疗法在SRCC患者中的疗效相对较差，与具有典型癌症病理特征的患者相比，总生存率较低。 SRCC癌症通常是在疾病晚期被诊断出来的，因此，与非标志性癌症相比，肿瘤通常具有更强的扩散性，这给治疗带来了挑战。将来，案例研究表明，骨髓转移可能会在印​​戒细胞胃癌的诊断和管理中发挥更大的作用。

在胃的SRCC中，去除胃癌是治疗的选择。没有任何一种化学疗法明显优于其他化学疗法，但最有效的疗法包括5-氟尿嘧啶（5-FU），顺铂和/或依托泊苷。一些较新的药物，包括紫杉醇和吉西他滨（Gemzar）正在研究中。

在对具有复发转移的膀胱SRCC患者的单例研究中，该患者表现出对姑息性FOLFOX-6化疗的治疗反应。

有报道称偶发性肺肿瘤血栓形成性微血管病可导致肺动脉高压和凝血问题，可用伊马替尼可成功治疗。

器官预后
这些侵袭性肿瘤通常被诊断为晚期，并且生存期通常较短。 SRCC的预后及其对特定治疗方案的化学敏感性仍存在争议，因为在大多数研究中尚未明确鉴定SRCC，其预后较差可能是由于其晚期。一项研究表明，其不良预后似乎是由其固有的肿瘤生物学引起的，为进一步研究提供了一个领域。

膀胱
原发性膀胱印戒细胞癌极为罕见，患者生存率很差，主要发生在38岁至83岁的男性中。但是，一名接受了根治性膀胱切除术并联合S-1和顺铂辅助化疗的患者证明有希望的90个月长期生存。

结直肠
结肠和直肠原发性印戒细胞癌（PSRCCR）很少见，据报道发病率不到1％。它的预后很差，因为症状通常会发展得较晚，而且通常会在晚期进行诊断。在以前的研究中，五年生存率从9％到30％不等。平均生存时间在20到45个月之间。它倾向于以较高的淋巴细胞侵袭率影响较年轻的成年人。值得注意的是，SRCC患者的总体生存率明显低于粘液性或分化差的腺癌患者。
