PCOS的常见体征和症状包括：

原因

诊断

标准评估

腹腔镜检查可以显示卵巢的增厚，光滑，珍珠白的外表面。（如果出于某些其他原因进行腹腔镜检查，这通常是偶然发现，因为以这种方式检查卵巢以确认PCOS的诊断是不常规的。）
血清（血液）雄激素水平（与雄性发育相关的激素），包括雄烯二酮和睾酮可能会升高。脱氢表雄酮硫酸盐水平高于700-800μg/ dL高度提示肾上腺功能障碍，因为DHEA-S仅由肾上腺制成。游离睾酮水平被认为是最好的措施，约60％的PCOS患者表现出超常水平。睾酮与性激素结合球蛋白（SHBG）比例的游离雄激素指数（FAI）很高，并且意味着是游离睾酮的预测因子，但是这是一个很差的参数，并且不比单独的睾酮更好。 PCOS的标志物，可能是因为FAI与肥胖程度相关。

相关条件
禁食生化屏和脂质谱
对有风险因素（肥胖，家族史，妊娠糖尿病史）的女性进行2小时口服葡萄糖耐量试验（GTT）可能表明15-33％的PCOS女性葡萄糖耐量受损（胰岛素抵抗）。在这种情况下，65-68％的女性可以看到弗兰克糖尿病。[引证需要]在正常体重和超重人群中都可以观察到胰岛素抵抗，尽管在后者中更常见（并且在更严格的NIH标准中更常见）用于诊断）; 50-80％的PCOS患者可能在某种程度上具有胰岛素抵抗。

根据一项前瞻性对照试验，葡萄糖耐量试验（GTT）代替空腹血糖可以增加PCOS患者糖耐量受损和明显糖尿病的诊断。[70]虽然空腹血糖水平可能保持在正常范围内，但口服葡萄糖测试显示，多达38％的无症状女性PCOS（一般人群中为8.5％）实际上患有葡萄糖耐量受损，根据ADA指南，有7.5％患有明显糖尿病的人。

鉴别诊断

管理

在每个领域中，关于最佳治疗存在相当大的争议。其中一个主要原因是缺乏比较不同治疗方法的大规模临床试验。较小的试验往往不太可靠，因此可能产生相互矛盾的结果。

有助于减轻体重或胰岛素抵抗的一般干预措施可能有益于所有这些目标，因为它们解决了被认为是潜在原因的问题。

由于PCOS似乎会引起严重的情绪困扰，因此适当的支持可能是有用的。

饮食
PCOS与超重或肥胖有关，成功减肥是恢复正常排卵/月经的最有效方法，但许多女性发现很难达到并维持显著的减肥效果。 2013年的科学评论发现体重和身体成分的类似减少以及妊娠率，月经规律性，排卵，雄激素过多症，胰岛素抵抗，脂质和生活质量的改善与体重减轻无关，与饮食构成无关。尽管如此，低GI饮食，其中大部分碳水化合物来自水果，蔬菜和全谷物来源，导致比常量营养素匹配的健康饮食更大的月经规律。

药物

如果不需要常规的月经周期，则不一定需要针对不规则周期的治疗。大多数专家说，如果月经出血至少每三个月发生一次，那么子宫内膜（子宫内膜）就会经常脱落，以防止子宫内膜异常或癌症的风险增加。如果月经不常发生或根本不发生，建议使用某种形式的孕激素替代品。另一种方法是间隔（例如，每三个月）服用口服孕激素以诱导可预测的月经出血。

替代药物

预测
