原发性硬化性胆管炎

原发性硬化性胆管炎(PSC)是肝脏和胆囊的长期进展性疾病,其特征在于胆管的炎症和瘢痕形成,其通常允许胆汁从胆囊排出。受影响的个体可能没有症状或可能出现肝脏疾病的体征和症状,例如皮肤和眼睛的黄色变色,瘙痒和腹痛。

体征和症状
近一半的PSC患者没有症状,并且由于肝功能检查异常而偶然发现PSC [1],但很大一部分患者会出现衰弱症状和症状。[7] PSC的体征和症状可能包括严重的瘙痒和非特异性疲劳。 还可以看到皮肤的黄化和眼睛的白色部分。 大约40%的受影响个体可见扩大肝脏和脾脏。 腹痛会影响约20%的PSC患者。

由于胆管引流受损,可以看到多次危及生命的急性胆管炎(胆管内感染),这增加了感染的风险。[8]

原因
原发性硬化性胆管炎的确切原因尚不清楚,其发病机制尚不清楚。[1]尽管PSC被认为是一种自身免疫性疾病,但它并未表现出对免疫抑制剂的明确反应。因此,许多专家认为它是一种复杂的,多因素的(包括免疫介导的)疾病,可能包含几种不同的肝胆疾病。[10] [11]

病理生理学

PSC通常基于在排除硬化性胆管炎的继发性原因后排除三种临床标准中的至少两种来诊断:

大多数患有PSC的人都有自身抗体和免疫球蛋白水平异常的证据。[20]例如,大约80%的PSC患者具有核周抗中性粒细胞胞浆抗体(P-ANCA);然而,这种免疫球蛋白和其他免疫球蛋白的发现并不是特异于PSC的那些,并且具有不明确的临床意义/后果。在20%-50%的PSC患者中发现抗核抗体和抗平滑肌抗体,同样,这些抗体不是特异性疾病,但可能鉴定也患有自身免疫性肝炎的PSC患者亚组(即PSC-AIH重叠综合征) [4]。

可以测量和监测的其他标志物是全血细胞计数,血清肝酶,胆红素水平(通常显著升高),肾功能和电解质。当吸收不良(例如,总脂肪泻)的症状突出时,偶尔会订购粪便脂肪。

管理

对PSC症状的支持性治疗是管理的基石。这些疗法旨在缓解诸如用止痒剂(例如胆汁酸螯合剂如消胆胺)引起的瘙痒等症状;抗生素治疗上行性胆管炎发作;和维生素补充剂,因为PSC患者通常缺乏脂溶性维生素(维生素A,维生素D,维生素E和维生素K)。[28]

也可能需要ERCP和专门技术来帮助区分良性PSC狭窄和胆管癌(胆管癌)。[29]

肝移植是PSC唯一经证实的长期治疗方法。移植适应症包括复发性细菌性上行性胆管炎,失代偿期肝硬化,肝细胞癌,肝门部胆管癌和门静脉高压症的并发症。并非所有患者都是肝移植的候选者,有些患者会在之后出现疾病复发。[10]

预测
从诊断到肝移植或PSC相关死亡的估计中位生存期为21。3年。[30]已经开发了各种模型来帮助预测存活,但是它们的使用通常最适合于研究而不是临床目的。血清碱性磷酸酶低于正常上限的1.5倍与更好的结果相关,但其在预测长期结果方面的效用尚不清楚。[1]

相关疾病
与PSC相关的任何癌症的发展预示着不良预后。 PSC相关癌症的并发症占PSC死亡人数的40%[2]。原发性硬化性胆管炎是胆管癌的主要已知危险因素之一,[31]胆管癌,其PSC患者的终生风险为10-15%。[3]与普通人群相比,这表明发生胆管癌的风险高出400倍。[1]鼓励对PSC患者胆管癌进行监测,一些专家建议通过专门的影像学检查和血清标志物进行年度监测[32],尽管尚未就模式和间隔达成共识。同样,筛查结肠镜检查对于接受新的原发性硬化性胆管炎诊断的人,建议使用它,因为他们患结肠直肠癌的风险是普通人群的10倍。[1]

流行病学
原发性硬化性胆管炎中有2-3:1的男性对女性偏好。[18] PSC可以影响任何年龄的男性和女性,尽管它通常在生命的第四个十年被诊断出来,最常见的是炎症性肠病(IBD)。[17] PSC进展缓慢并且通常是无症状的,因此它可以在诊断之前存在多年并且在其导致临床上显著的后果之前存在。关于原发性硬化性胆管炎的患病率和发病率的数据相对较少,不同国家的研究显示,每10万人的年发病率为0.068-1.3,流行率为每100,000人0.22-8.5;鉴于PSC与溃疡性结肠炎密切相关,在UC更常见的人群中风险可能更高。[35]在美国,估计有29,000人拥有PSC。[1]
