肺栓塞

体征和症状

风险因素

根本原因

诊断

概率测试

肺栓塞排除标准(PERC)有助于评估疑似肺栓塞的人,但不太可能。与Wells评分和日内瓦评分不同,这些临床预测规则旨在对疑似PE患者进行分层,PERC规则旨在排除人们在将医生分为低风险类别时患PE的风险。

没有任何这些标准的低风险类别的人可能不会进行PE的进一步测试:低氧饱和度 -SaO2 <95%,单侧腿部肿胀,咳血,先前DVT或PE,近期手术或创伤,年龄> 50,激素使用,心率快。这一决定背后的理由是,进一步的检测(特别是胸部的CT血管造影)可能比PE的风险造成更大的伤害(来自辐射暴露和对比染料)。 PERC规则的敏感性为97.4%,特异性为21.9%,假阴性率为1.0%(16/1666)。

验血

在不知道PE高风险的典型人群中,在使用更简单的一线测试后,成像有助于确认或排除PE的诊断。 医学协会建议诸如D-二聚体等测试首先提供成像需要的支持证据,如果其他测试证实中等或高概率找到支持诊断PE的证据,则会进行成像。

CT肺动脉造影是大多数人推荐的一线诊断成像检查。

从历史上看,诊断的黄金标准是肺血管造影,但随着非侵入性技术的增加,这已被废弃。腿的超声可以确认PE的存在,但不能排除它。

CT肺动脉造影
CT肺血管造影(CTPA)是使用计算机断层扫描(CT)与放射性对比而不是右心导管插入术获得的肺血管造影。它的优点是临床等效性,非侵入性,对人的更大可用性,以及在没有肺栓塞的情况下从鉴别诊断中识别其他肺病的可能性。

多排CT(MDCT)机器中可用检测器行数的快速变化阻碍了CT肺动脉造影的准确性评估。根据一项队列研究,单层螺旋CT可能有助于诊断疑似肺栓塞患者的检测结果。在这项研究中,敏感性为69%,特异性为84%。本研究中检出率为32%,阳性预测值为67.0%,阴性预测值为85.2%。然而,由于CT扫描是肺栓塞患者的最终诊断工具,因此该研究的结果可能由于可能的并入偏差而存在偏差。作者指出,阴性单层CT扫描不足以排除肺栓塞本身。一项单独的研究,使用4片和16片扫描仪的混合物报告了83%的敏感性和96%的特异性,这意味着如果在成像中没有看到它是排除肺栓塞的良好测试,并且它如果看到肺栓塞,则非常擅长确认肺栓塞。该研究指出,当临床概率与成像结果不一致时,还需要进行额外的检测。与VQ扫描相比,CTPA不逊色于VQ扫描,并且识别出更多的栓子(不一定能改善结果)。

胸部X光通常在呼吸急促的人身上进行,以帮助排除其他原因,如充血性心力衰竭和肋骨骨折。 PE中的胸部X射线很少正常,但通常缺乏表明PE诊断的迹象(例如,Westermark征,Hampton驼峰)。

心电图

超声心动图

预防
进一步的信息:预防血栓形成
有风险因素的人可能会预防肺栓塞。住院的人可能会接受预防性药物治疗,包括普通肝素,低分子肝素(LMWH)或磺达肝癸钠,以及抗血栓形成的长袜,以减少腿部可能移位并迁移到肺部的DVT风险。

在先前的PE患者完成华法林治疗后,长期服用阿司匹林有助于预防复发。

治疗
抗凝治疗是主要的治疗方法。急性地,可能需要支持性治疗,例如氧气或镇痛。人们经常在治疗的早期阶段住院,并且在INR达到治疗水平之前往往会继续接受住院治疗。然而,越来越多的低风险病例在家中以DVT治疗中常见的方式进行管理。 支持一种方法与另一种方法的证据很薄弱。

抗凝

同样,孕妇通常在分娩后至少六周内服用低分子量肝素,以避免华法林的已知致畸作用,特别是在怀孕的早期阶段。

华法林治疗通常持续3-6个月,如果之前存在DVT或PE,则“终身”,或者没有通常的危险因素。治疗结束时异常的D-二聚体水平可能表明首次无端肺栓塞患者需要继续治疗。对于那些体重较小的PE(称为亚段PE),抗凝作用的效果尚不清楚,因为截至2014年尚未对其进行适当研究。

溶栓

在非大规模PE中使用溶栓治疗仍有争议。 有些人发现,这种治疗可以降低死亡风险并增加出血风险,包括颅内出血。 其他人发现死亡风险没有下降。

下腔静脉滤器

有两种情况下,下腔静脉滤器被认为是有利的,如果禁忌抗凝治疗(例如在主要手术后不久),或者一个人尽管有抗凝治疗仍有肺栓塞。在这些情况下,可植入以防止新的或现有的DVT进入肺动脉并与现有的阻塞相结合。尽管该装置具有预防肺栓塞的理论优势,但缺乏支持其有效性的证据。

手术

流行病学
每年在美国有超过60万人患有肺栓塞。它导致美国每年有50,000 到200,000人死亡。住院患者的风险约为1%。在过去的25年中,美国的致命肺栓塞率从6%下降到2%。

预测

预后取决于受影响的肺量和其他医疗条件的共存;慢性肺部栓塞可导致肺动脉高压。在大量PE后,如果患者要生存,必须以某种方式解决栓子。在血栓性PE中,血凝块可以通过纤维蛋白溶解分解,或者可以组织和再通,以便通过凝块形成新的通道。 PE后第一天或第二天血流量恢复得最快。此后改善进展缓慢,一些赤字可能是永久性的。关于小的亚段PE是否需要治疗存在争议,并且有证据表明,亚段PE患者可能在没有治疗的情况下表现良好。

一旦停止抗凝治疗,致命性肺栓塞的风险为每年0.5%。

据称未经治疗的PE的死亡率为26%。这个数字来自于Barrit和Jordan于1960年发表的一项试验,该试验将抗凝与安慰剂进行比较,用于治疗PE。 Barritt和Jordan于1957年在布里斯托尔皇家医院进行了这项研究。这项研究是迄今为止唯一一项检查抗凝血剂治疗PE的安慰剂对照试验,其结果令人信服,以至于该试验从未重复过这样做会被认为是不道德的。也就是说,安慰剂组报告的死亡率为26%可能是一种夸大其辞,因为当天的技术可能只检测到了严重的PE。

预测死亡率
