癌细胞

癌细胞是不断分裂的细胞,形成实体肿瘤,或使血液充满异常细胞。细胞分裂是人体生长和修复的正常过程。一个父细胞分裂成两个子细胞,这些子细胞被用来构建新的组织,或者用来替换因衰老或损伤而死亡的细胞。当不再需要更多的子细胞时,健康细胞就停止分裂,而癌细胞则继续复制。它们也能从身体的一个部位扩散到另一个部位,这个过程被称为转移。

分类
存在不同类别的癌细胞,根据它们起源的细胞类型来定义。

癌症,大多数癌细胞起源于上皮,起始于排列在身体表面的膜组织。
白血病起源于负责产生新血细胞的组织,最常见于骨髓中。
淋巴瘤和骨髓瘤,来自免疫系统的细胞。
肉瘤,起源于结缔组织,包括脂肪,肌肉和骨骼。

癌细胞具有区别于显微镜下可见的组织学特征。 细胞核通常较大且不规则,细胞质也可能表现出异常。


核的形状,大小,蛋白质组成和质地通常在恶性细胞中改变。 细胞核可能获得凹槽,褶皱或凹陷,染色质可能聚集或分散,核仁可能变大。 在正常细胞中,细胞核通常呈圆形或椭圆形,但在癌细胞中,轮廓通常是不规则的。 不同的异常组合是不同癌症类型的特征,在某种程度上核外观可用作癌症诊断和分期中的标记物。

当负责调节细胞分裂的基因受损时,会产生癌细胞。癌变是由正常细胞的遗传物质的突变和表观突变引起的,这破坏了增殖和细胞死亡之间的正常平衡。这通过体内的自然选择导致不受控制的细胞分裂和这些细胞的进化。细胞不受控制且通常快速增殖可导致良性或恶性肿瘤(癌症)。良性肿瘤不会扩散到身体的其他部位或侵入其他组织。恶性肿瘤可侵入其他器官,扩散到远处(转移)并危及生命。

癌症发生需要一个以上的突变。事实上,在正常细胞转化为癌细胞之前,通常需要对某些类别的基因进行一系列突变。

暴露于辐射,化学物质和其他环境来源可能会对DNA造成损害,但由于DNA转录中未经纠正的错误,突变也会随着时间的推移自然积累,从而使年龄成为另一个风险因素。肿瘤病毒可引起某些类型的癌症,并且已知遗传学也起作用。

干细胞研究表明,过多的SP2蛋白可能会使干细胞转变为癌细胞。然而,缺乏特异性共刺激分子有助于抗原与淋巴细胞反应的方式可能损害自然杀伤细胞的功能,最终导致癌症。

病理
在免疫系统中起作用的细胞,例如T细胞,被认为在确定是否杀死病态或受损的人类细胞时使用双受体系统。如果细胞处于压力下,变成肿瘤或被感染,则会产生包括MIC-A和MIC-B在内的分子,这样它们就可以附着在细胞表面。这些工作可以帮助巨噬细胞检测并杀死癌细胞。

发现
人类癌症的早期证据可以从埃及论文(公元前1538年)和木乃伊化遗骸中解释。 2016年,Edward John Odes(来自南非Wits医学院的解剖科学博士生)及其同事报告了170万年前的骨肉瘤,代表了最古老的恶性人类癌症。

在文艺复兴时期和发现时代,对癌症的理解显著提高。德国生物学家和政治家鲁道夫·维尔乔爵士研究了微观病理学,并将他的观察与疾病联系起来。他被描述为“细胞病理学的创始人”。 1845年,Virchow和John Hughes Bennett独立观察患者白细胞异常增加。 Virchow正确地将这种病症确定为血液病,并于1847年将其命名为leukämie(后来被证实为白血病)。 1857年,他是第一个描述一种叫做脊索瘤的肿瘤,起源于斜坡(位于颅底)。
